Pulmonary Critical Care and Sleep Medicine

The department of Pulmonary Medicine made a humble beginning in 1991 for specialized, tertiary care of patients with Respiratory ailments with 10 ward beds, and four bedded ICU and the first sleep lab of India. Over the years, department expanded to management of various pulmonary diseases including asthma, tuberculosis, lung cancer and intestinal lung disease; besides critical care, bronchoscopy, thoracoscopy and sleep lab facilities. The department expanded bronchoscopic facilities with the addition of Endobronchial ultrasound (redial plus linear EBUS) in year 2013.

The department has been involved in the academic activities with a prestigious DM Courses in Pulmonary & Critical Care Medicine since year 2010. After initial three years the seats for DM course were increased from two to four per year and top rankers across the country get enrolled on an annual basis.

In year 2018 the department shifted to Super Speciality Block – 6th floor and the facilities were expanded to 10 bedded ICU, 12 bed HDU for H1N1 & 18 bedded ward for patients, three sleep labs, one dedicated spirometry lab, daily OPDs and daily bronchoscopy. The department also runs a 24hrs ABG lab in the Department as well as in the emergency (NEB).

The department currently has seven faculty, alongwith a team of DM Residents, Senior Residents and Junior Residents. Besides carrying out regular academic teaching schedule for the residents department is also involved in Government healthcare programs such as Dengue, Swine flu and workshop on management of H1N1 influenza patients. Department also imparted training for the Medical officers on route of Amarnath Yatra, made curricular and training content for sensitized on severe respiratory disease associated with MERS (CoV). Formulated National Guidelines/ protocol on H1N1 influenza, pneumonia, sleep apnea and COVID 19, developed guidelines for vaccination of health care workers, CPAP, Bipap and oxygen concentrator, formulation of nebulization practices in India. They are a part of the central team who lead the management of H1N1, Encephalitis, COVID 19 in different states of India.

A new ten (10) bedded ICU has been set up in the new location of the department for care of patients with acute respiratory ailments and other critically sick patients with all modern facilities like monitors, ventilators mechanical (invasive & non- invasive) including high frequency ventilator, infusion pumps, blood warmers, body warmers, bedside ultrasound machine etc. There is an effort to also procure continuous renal replacement therapy equipment, counter pulsation balloon pumps among others for the ICU for comprehensive management of critically ill patients. Back up services like radiology and laboratory services are available round the clock.

The DM & non DM Senior residents manning the critical care unit are trained in all interventional procedures like placement of central lines, endotracheal intubations, and intercostal intubation among others. They are helped in the care by a team of junior residents and nursing staff

The department also runs a 06 bedded ICU and of 06 bed HDU for H1N1 patients in the Department. Facilities and staffing of care personnel is akin to that of the Pulmonary Medicine ICU.

OPD & IPD patient admission, all diagnostic and therapeutic procedures like bronchoscopy, EBUS, pulmonary function tests, intercostal intubations, pleurodesis, percutaneous tracheostomy etc. The department also provides round the clock back up for acute respiratory emergencies and critical illness of all disciplines including those from the Casualty department. In the OPD, like all other specialty units patients are registered when they are referred from a primary care unit of the hospital, CGHS dispensaries or from other hospitals. The OPD is run on all working days. Registration is open from 08.30 am to 11.30 am on all days except on Saturdays when it is till 11.00 am. OPD patient attendence = New-14665, old-18320. Inpatient: The department has bed strength of 40 beds (18 ward + 12 HDU + 10 ICU). Indoor beds are for care of patients with respiratory ailments requiring hospitalization. Patients with respiratory ailments requiring non- invasive ventilation, oxygen, parenteral medication, nebulization therapy and/ or monitoring are admitted to the pulmonary care ward/ HDU. For the care of these patients the faculty is ably assisted by DM Residents (12), Non-DM SR (10) and junior residents (10) who are posted in shifts.

The department performs all diagnostic and therapeutic procedures related to the management of respiratory disease like bronchoscopy, EBUS, TNBA, pulmonary function tests, ABG, intercostal intubations, pleurodesis, percutaneous tracheostomy etc. The department also provides round the clock back up for acute respiratory emergencies and critical illness of all disciplines including those from the Casualty department. The department independently manages critical care patients in 10 bedded ICU. Pulmonary Department of Safdarjung Hospital is pioneer in establishing Sleep Medicine department with 1st Sleep Lab in Govt. Hospital. The department has provided teaching training including courses like DNB (Pulmonary Medicine), Fellowship in Critical Medicine (FNB) is centre for DM (PCCM) since 2011

Diagnostic and Therapeutic Facilities: The department offers state of art diagnostic facilities to aid in the diagnosis and management of inpatients and out patients. They include:

Pulmonary Function Laboratory: The Department has machines for spirometry, complete pulmonary function test machines including diffusion capacity.

The department has procured three pulmonary function machines and four protable spirometers for measurement of lung volumes, thoracic gas volumes, diffusion factor, body plethysmography for measurement of airway resistance, compliance and thoracic gas volumes etc. The department is in the process of procuring FENO & Impulse oscillometery apparatus.

Pulmonary function tests are done on daily between 9.00 am to 4.00 pm (Saturday till 1 pm) in room no. 609.

Bronchoscopy Unit: The department has a bronchoscopy unit with adult and pediatric fiber bronchoscopes. All Bronchoscopy diagnostic procedures are performed routinely, viz- bronchial washing for cytology & microbiology, brushing for cytology, bronchoscopic/ transbronchial lung biopsy & trans bronchial needle aspiration. The department has a semi rigid medical thoracoscopy system for medical thoracoscopies. The department has an End bronchial Ultra Sound facility for evaluation of Mediastinal Lymphadenopathy along with a Radial probe for Ultra sound guided evaluation of peripheral lung masses & central tracheal abnormalities under Videoscopic guidance.

Bronchoscopies for removal of foreign bodies are also done. The department also has a rigid bronchoscope which is expected to aid significantly in the diagnostic and therapeutic bronchoscopic facilities.

Several new therapeutic bronchoscopic maneuvers like Balloon dilatation of bronchial strictures, electro cauterization of bronchial tumors, placement of bronchial stents and pleurodesis under vision is being undertaken.

Bronchoscopies are performed on appointment between 8.30 am to 2.00 pm. This is facility with nominal payments. Bronchoscopes are done in room No. 645.

ABG Laboratory: The department runs the central arterial blood Gas Test Laboratory. The Laboratory having two machines caters to the ABG requirements of the whole hospital and the services are available 24x7 round the clock in room no 616.

The Department also runs the ABG Laboratory located in the New Emergency Block in Room No 103. This facility too is a 24x7 facility and does approximately 400 ABG samples daily.

Sleep Laboratory: Safdarjang Hospital and the department of pulmonary medicine are credited with setting up the first sleep laboratory in India. Here patients undergo overnight polysomnography for the diagnosis and possible therapeutic options for treatment of sleep disorders. This path breaking step has practically opened up a new and dynamic sub specialty in sleep medicine. Sleep studies are done in room 662.

Besides the above, diagnostic procedures like pleural fluid aspiration, pleural biopsies, fine needle aspiration cytology – routine or ultrasound/CT guided are done routinely on an OPD basis.

The department has hosted the first National Conference (NAPCON) of Chest Physicians in 1999 with the combined presence of the ICS (Indian Chest Society) and NCCP (National College of Chest Physicians, supported by the ACCP (American Colleges of Chest Physician), ATS (American Thoracic Society) and ASDA (American Sleep disorders Association).

The department has also hosted the first ever international conference on sleep medicine in 1995 in coordination with the (ASDA) American Sleep Disorders Association. Since then the department has hosted several international conferences on sleep medicine. These have given rise to an interest among physicians in a hitherto unexplored field of medicine.

The Department regularly holds case based discussion (PCCM) conferences annually.

The Department also holds PFT workshop in coordination with VMMC, MBBS students annually.

  • Since September 2012 the first batch of students (TWO) in DM (Pulmonary & Critical Care) under the Guru Govind Singh Indraspratha University joined the Department, the course is for three years. As of June 2013, the number of DM students admitted for DM & Pulmonary Critical Care has been enhanced to 4 per year. Therefore as on date there are twelve (12) DM Residents.
  • Like all other units the department trains interns/ externs. Post graduate students from other institutions have been coming on a regular basis for training in respiratory medicine & critical care.
  • Learning is a continuous process in the department with weekly seminars to keep the faculty and staff abreast with the latest developments.
  • The department is recognized as a center for examinations of DNB in respiratory medicine.
